To make a placement referral for the Intact Projects Ltd we also require you to complete a Placement Risk Assessment to be completed by an appropriate professional who knows the young person.

This document is crucial as it also provides invaluable information about the young person we need to know BEFORE they are placed with us. The Placement Risk Assessment is also required from us by our insurers.

Preferably, complete the Placement Risk Assessment by using the online form. This will be sent to us automatically when you press SUBMIT.

A member of staff will contact you the same day, alternatively, you may call to discuss details prior to our response.

All New placements out of boundary will be notified to the home local authority from the outset, they will also be advised of discharge from our service.
